Bridging the gap program
As an extension of our dedication to the ongoing development and support of diabetes management, Medtronic is pleased to offer the Medtronic Bridging the Gap Program.
Who is eligible?
To be eligible, you must:
- Have private health cover
- Be in the interim 12-month waiting period for private health cover benefits
- Have been diagnosed with Insulin Dependent diabetes
- Meet the NDSS criteria for insulin pump consumables. For details, visit NDSS.
- Be under supervision from healthcare professionals, including a diabetes educator and endocrinologist or paediatrician, for insulin pump therapy.
Applying for the Medtronic Bridging the Gap Program
How to Apply?
To apply for a loan pump, simply complete the application form, which sets out the terms and conditions of our program. The form also requires signed acknowledgement from a physician and a diabetes educator prior to submission.
Receiving a Medtronic loan pump
Successful applicants will have access to a Medtronic insulin pump* for the remainder of their health fund waiting period (a maximum of 12 months).
Medtronic Diabetes will confirm an applicant’s details upon receipt of the completed form. Loan pumps will only be delivered to diabetes education centres or hospitals and patients will need to make contact with the respective centre to arrange an appointment for training on a loan pump. Upon completion of the health fund waiting period, the individual will be required to return the loan pump to Medtronic.
